SHILOH FITNESS MEMBERSHIP AGREEMENT, LIABILITY WAIVER, & PRIVACY POLICY
Welcome to the Shiloh Fitness community! Please read and sign below before using our facility—whether you’re a member, guest, or here for a trial. This agreement applies to all members, guests, trial users, day pass holders, week pass holders, personal training clients, class participants, event participants, and any other users of Shiloh Fitness facilities, programs, or services, whether on-site or off-site.

Key Terms

Memberships are billed on the 1st of each month and renew automatically until canceled.

You may cancel at any time. No refunds or prorated credits will be issued for unused days in the current billing period.

Family memberships: All members under 16 must be supervised by an adult member at all times.

Day Pass, Week Pass, and Trial guests must agree to these terms and sign this waiver.
Rules & Code of Conduct

Treat everyone with respect—no harassment, discrimination, or disruptive behavior.

Wear proper attire and footwear at all times.

Children under 16 must be supervised by an adult member at all times.

Sauna use is restricted to members 18 years and older. Minors may only use the sauna with prior written permission from Shiloh Fitness management. Permission may be granted or denied at the sole discretion of the facility for safety reasons. Use is at your own risk.

Use equipment safely and follow all posted instructions.

Facility rules may be updated at any time and are posted in the gym.
Payment Authorization
I authorize Shiloh Fitness LLC to charge my payment method on the 1st of each month for membership fees and any other applicable charges, recurring as described above.
Liability Waiver & Release

I am voluntarily participating in activities at Shiloh Fitness, located in the Global Spheres Center (GSC).

I understand that physical activity, personal training, group fitness classes, specialty programs, events, use of equipment, use of facilities, and sauna use involve inherent risks of injury, illness, or worse.

I use all facilities at my own risk.

I waive and release Shiloh Fitness LLC, its affiliates, parent companies, subsidiaries, managers, members, officers, employees, contractors, agents, landlords, property owners, and facility partners, including Harvest Ventures LLC of Texas, Global Spheres Center and Glory of Zion International Ministries, Inc. (GZI), from any and all liability for injury, loss, or damage to myself or my property, except as otherwise prohibited by law.
If the Member is a Minor (Under 18)
By signing this agreement, I certify that I am the parent or legal guardian of the minor named on this membership and have full legal authority to enter this agreement on their behalf.
I understand and agree:

All minors under 16 must be supervised by an adult member at all times.

Sauna use is prohibited for anyone under 18 unless special written permission is granted by Shiloh Fitness management. Any approved minor sauna use requires parent/guardian consent and compliance with all sauna safety guidelines.

Personal Training Eligibility:

Ages 12–15 may participate only with parent/guardian present and trainer approval.

Ages 16–17 may participate with parent/guardian consent, which is granted by signing this agreement.

I accept all risks on behalf of the minor and agree that they will follow all rules, age restrictions, and staff instructions.

I waive and release Shiloh Fitness LLC, its affiliates, parent companies, subsidiaries, managers, members, officers, employees, contractors, agents, landlords, property owners, and facility partners, including Harvest Ventures LLC of Texas, Global Spheres Center and Glory of Zion International Ministries, Inc. (GZI), from any and all liability for injury, loss, or damage to myself or my property, except as otherwise prohibited by law.
Photo/Video Release (Optional Opt-Out)
Shiloh Fitness occasionally captures photos or videos during workouts, classes, and events to promote our programs and highlight member achievements.
Unless you indicate otherwise below, you grant permission for Shiloh Fitness to use images or video of you or your minor child taken in the facility or at any Shiloh Fitness–sponsored event for promotional or informational purposes (digital or print).
I do NOT consent to the use of my or my child’s image, likeness, or video by Shiloh Fitness.

At Shiloh Fitness, we value your privacy. This policy explains what information we collect, how we use it, and your rights regarding your personal data.

CANCELLATION AND REFUND NOTICE (TEXAS HEALTH SPA ACT)
NOTICE TO PURCHASER: DO NOT SIGN THIS CONTRACT UNTIL YOU READ IT OR IF IT CONTAINS BLANK SPACES.
IF YOU DECIDE YOU DO NOT WISH TO REMAIN A MEMBER OF THIS HEALTH SPA, YOU MAY CANCEL THIS CONTRACT BY MAILING TO THE HEALTH SPA BY MIDNIGHT OF THE THIRD BUSINESS DAY AFTER THE DAY YOU SIGN THIS CONTRACT A NOTICE STATING YOUR DESIRE TO CANCEL THIS CONTRACT. THE WRITTEN NOTICE MUST BE MAILED BY CERTIFIED MAIL TO THE FOLLOWING ADDRESS:

IF THE HEALTH SPA GOES OUT OF BUSINESS AND DOES NOT PROVIDE FACILITIES WITHIN 10 MILES OF THE FACILITY IN WHICH YOU ARE ENROLLED OR IF THE HEALTH SPA MOVES MORE THAN 10 MILES FROM THE FACILITY IN WHICH YOU ARE ENROLLED, YOU MAY:
(A) CANCEL THIS CONTRACT BY MAILING BY CERTIFIED MAIL A WRITTEN NOTICE STATING YOUR DESIRE TO CANCEL THIS CONTRACT, ACCOMPANIED BY PROOF OF PAYMENT ON THE CONTRACT TO THE HEALTH SPA AT THE FOLLOWING ADDRESS:

(B) FILE A CLAIM FOR A REFUND OF YOUR UNUSED MEMBERSHIP FEES AGAINST THE BOND OR OTHER SECURITY POSTED BY THE HEALTH SPA WITH THE TEXAS SECRETARY OF STATE. TO MAKE A CLAIM AGAINST THE SECURITY, PROVIDE A COPY OF YOUR CONTRACT TOGETHER WITH PROOF OF PAYMENTS MADE ON THE CONTRACT TO THE TEXAS SECRETARY OF STATE. THE REQUIRED CLAIM INFORMATION MUST BE RECEIVED BY THE SECRETARY OF STATE NOT LATER THAN THE 90TH DAY AFTER THE DATE NOTICE OF THE CLOSURE OR RELOCATION IS FIRST POSTED ON THE SECRETARY OF STATE’S INTERNET WEBSITE.
IF YOU DIE OR BECOME TOTALLY AND PERMANENTLY DISABLED AFTER THE DATE THIS CONTRACT TAKES EFFECT, YOU OR YOUR ESTATE MAY CANCEL THIS CONTRACT AND RECEIVE A PARTIAL REFUND OF YOUR UNUSED MEMBERSHIP FEE BY MAILING A NOTICE TO THE HEALTH SPA STATING YOUR DESIRE TO CANCEL THIS CONTRACT. THE HEALTH SPA MAY REQUIRE PROOF OF DISABILITY OR DEATH. THE WRITTEN NOTICE MUST BE MAILED BY CERTIFIED MAIL TO THE FOLLOWING ADDRESS:

Conclusion and Signature
By signing, I acknowledge that I have read, understand, and agree to be bound by this Membership Agreement, Liability Waiver, Privacy Policy, and all terms contained herein.
